www.kids-ot is a partnership between Occupational Therapists Robyn Sims and Mary Rydstrom for the provision of group therapy programs and information sessions.

These include phone or email conversations, face to face interactions, and interactions through our website and during the provision of services. We may also collect information through referrals, other service providers and others who are directly involved in the care of your child.
Client files and information are held in secure filing cabinets and / or electronic document management systems. Each file includes personal information including name, address, contact numbers and emails, relevant medical history including referring doctors, and other information relevant to the provision of occupational therapy services.
Purpose of holding personal information:
Personal information is gathered, stored and used for the purpose of providing appropriate occupational therapy services, including assessment, individual treatment or group programs. It is also used to appropriately invoice and receipt for services.
Some photographs may be taken during individual or group sessions. You will have signed an agreement in your initial interview form. These are for the express purpose of documenting sessions and sharing information with you as a parent. You will be contacted directly should there be a request to utilise any images of your child for any other purpose, unless you have provided written approval previously.
Personal information will not be disclosed to a 3rd party nor discussed unless:

- Disclose relevant information in any other way
- You would reasonably expect your personal information to be disclosed to another professional or agency such as your referring doctor, and this disclosure is related to the primary purpose for which your information is collected.
